Pays a lump sum benefit to an employee rendered totally & permanently disabled due to natural or accidental causes.
Advances half of the life benefit up to a maximum amount if employee is found to be terminally ill with a life expectancy of 12 months or less.
Provides a fixed amount of cash to your employee for each day of hospital confinement due to illness or accident.
Pays a pre-determined amount if employee is diagnosed with any critical illness covered in plan.
What does “pre-existing conditions" mean?
Any illness or condition occurring before the effective date of a Member’s insurance or the approval date of the latest Reinstatement, if any, which satisfy the following conditions: (a) any professional advice or treatment was given for such illness or condition; (b) such illness or condition was in any way evident to the Insured; or (c) the pathogenesis of such illness has started whether or not the Insured is aware of such illness or condition.
What do we mean by “contributory plans"?
A plan fully or partially funded by the employee and involves no mandatory contributions from the employer/company.
What do we mean by “dismemberment"?
Becoming disabled by losing a part of your body as a result of an accident.
GROUPS
Employer-Employee groups; Labor Unions; Cooperatives; Associations
Groups should have at least 26 eligible individuals comprising 100% of the group members
INSUREDS
For Employer groups: Regular, full-time and actively-at-work employees, ages 18 to 64
For Non-Employer groups: Bona fide members actively performing the daily normal chores of life, ages 18 to 64
Suicide is not included within the first 12 months of coverage.
For contributory plans, hospitalization due to illness will be covered 31 days after the insured's date of enrollment into the policy.
Pre-existing conditions may be contestable within the first 12 months of coverage.
